Report writing

Leeds Orthotics can provide reports suitable for a range of purposes. We write reports for individuals and organisations for medicolegal, second opinion and educational purposes. Reports can be compiled using input from across our experienced multi disciplinary team. Following an appointment, reports can be sent by email in PDF format for convenience.

Reports that we can provide include:

Report summarising initial assessment

We can provide a report following your initial assessment, detailing the findings of the clinical assessment and discussion and recommendations of our orthotists. This can be for your personal records or for your GP. Upon request, we can include recommended options for treatment.

Reports suitable for supporting applications for funding charities, NHS services and other funding organisations

Reports to be passed on to other organisations can be provided. This might detail how you could benefit from a particular orthotic intervention and the positive impact this could have on your day to day life, suitable for supporting a funding application to the NHS or a charity.

Reports detailing second opinions

If you have any doubt about advice that you have been given by a healthcare professional, or if you would like input from a second clinician then our orthotists can provide an impartial and independent opinion. A report can be produced to outline a second professional medical opinion.

Reports to support a child’s assessment for an Education, Health and Care Plan (EHCP)

Children and young adults upto the age of 25 who are in nursery or education, including further education may be eligible for an ECHP. Our clinicians are able to produce a report providing supporting evidence to inform an ECHP, outlining details such as frequency of physiotherapy appointments, recommended orthotic intervention or requirement for specialist footwear.

Case manager’s report

Reports detailing the cost of a rehabilitation package, estimated timescale for recovery or before return to work can be offered that are suitable to be provided to a client. We work with case managers to ensure that clients are able to achieve optimum levels of mobility in as short a time frame as possible. It may be necessary for your client to come in for a clinical assessment, that we can independently assess your patient’s needs.

Insurance report

We work with insurance companies to produce reports that indicate recommended treatment for patients, along with any appropriate further therapy and the associated costs of rehabilitation.

Reports to support referral to another service

We are able to provide reports which give details to support a referral to other services within our multidisciplinary team and partner services. These reports are also suitable for referral to other healthcare professionals or services which we do not offer, and should you wish to be treated elsewhere for any reason we are able to provide external referrals.

Reports to support communication between NHS and private services

Sometimes during your treatment, you may require a report for an NHS healthcare provider, GP or consultant and these can be provided. Information detailing assessments, reviews, progress and recommended courses of treatment will be included along with any other specific information required.
